To determine if you qualify for services provided by Grattan Private Duty Nursing, answer these questions:
- Are you currently receiving home health or Private Duty Nursing services?
- Do you have Minnesota Medical Assistance Insurance or are you receiving managed care through an insurance company or County Waivered Program on behalf of Minnesota Medical Assistance?
- Are you on a CAC waiver through your county?
